Chronic Disease Management (CDM) plans are available to people with a chronic or terminal medical condition that has been, or is likely to be, present for 6 months or longer.
CDM plans are created between a patient and their doctor to help coordinate care and provide a structured approach for the ongoing management their chronic or terminal condition.
These plans should be created with the GP you see most often and who you will continue to see most often over the next 12 months.
Examples of conditions that are likely to be eligible for a CDM plan include, chronic musculoskeletal conditions, asthma, diabetes, cardiovascular disease, kidney disease, and stroke.
When planning your chronic disease management, if you’re eligible, your GP might also suggest a GP Management Plan (GPMP) and Team Care Arrangements (TCAs). If your doctor has developed both a GPMP and TCAs for the management of your chronic condition, you may be eligible for Medicare rebates on up to five one-on-one visits per year with allied health professionals, for appointments directly related to your chronic condition. This includes allied health professionals such as, Diabetes Educators, Occupational Therapists, Audiologists, Physiotherapists, Psychologists, Aboriginal and Torres Strait Islander Health workers, Dietitians, and more.
